It will be necessary to have a written
eyeglass prescription that includes the pupillary distance (or PD) to
continue under this format. This
can be obtained from your eye care physician. Check with your
safety dept. to determine if your company has W2T order forms
for your use. If your company does not have the W2T order form,
you may click the link below to print them. The order form is in
the standard ".PDF" format which requires the Acrobat
Reader program. Most computer systems already have this software
installed. If you do not may download a FREE version by clicking

Decide on color and size by looking at
the wall chart, the samples provided are marked with the size, the next
bigger or smaller will be in 2mm increments. (the nosepiece will remain
the same, but the eyepieces will increase or decrease by 2mm in
circumference)
Single Vision
(no bifocal) Fax a copy of your prescription with any lens
options you require and frame selection. A follow up
phone
call is
required for confirmation and payment options.
Bifocal (with the line) wearers, you
must decide the type of bifocal you prefer
with lens option requirements. Fax a copy of your RX with the frame
name/color/size. A follow up phone call
is required for confirmation and
payment options.
Progressive
(without the
line) wearers, list any lens options you
prefer.

Your OSHA/ANSI approved safety glasses
will be shipped in approximately 2 weeks with detachable side shields in
an eyeglass case. The safety glasses will be adjusted for basic
alignment before they leave the lab, some minor adjustments may be
necessary after shipping. Your local optician can adjust your new
glasses if needed.
